The treatment request has to be submitted in German or English. The documents required for treatment requests have to be submitted in German or English language. Please provide the following information:

Personal data of the patient (full name, address, date of birth, availability, passport copy)

Diagnoses and, if known, required treatment, contact of previously treating physicians, if applicable

Previous findings (doctors’ letters, reports, imaging)

Note on the necessity of an interpreter if German or English is not spoken

Based on these documents, the UKGM staff will make a decision as to whether treatment appears reasonable and appropriate from a medical perspective. The decision will be communicated in writing upon completion of the assessment.
